Hypopharyngeal cancer is characterized by an aggressive clinical course, which necessitates the search for alternative and reliable markers for early diagnosis. Peroxidase, as a component of the antioxidant status, plays a dual role in carcinogenesis, influencing cell proliferation and apoptosis. To analyze the dynamics of peroxidase activity (specifically GPx1 and GPx4) as potential indicators of hypopharyngeal cancer aggressiveness. A comprehensive review of scientific literature published on the subject over the last 10 years was conducted. The relevance of the research topic was established, and a systematic literature review was performed. The core mechanisms of oxidative stress and their functional relationship with peroxidase activity were elucidated. A correlation between peroxidase levels and the stage of the cancerous process was identified. It was demonstrated that peroxidase activity can serve as both an indicator and a prognostic parameter for the clinical course of hypopharyngeal cancer. Furthermore, the factors underlying resistance to treatment modalities in hypopharyngeal cancer under conditions of high peroxidase activity were substantiated. The dynamics of peroxidase activity serve as an informative indicator of the stage of oncogenesis. High activity in the early stages reflects a protective response, whereas its decline acts as a prognostic marker of decompensation and poor clinical outcome. The peroxidase (PO) index is recommended for inclusion in patient monitoring algorithms during chemoradiotherapy
